Our Care Process
Physical disability support begins when a client, family member, or healthcare professional contacts us. During this initial consultation, we gather important information about the client’s condition, mobility level, daily living challenges, medical requirements, and personal care needs.
Our care team then conducts a comprehensive functional and medical assessment. This assessment evaluates physical mobility, muscle strength, balance, use of assistive devices, medication needs, skin integrity, and the safety of the home environment. We also assess the client’s ability to perform daily activities independently.
Based on this assessment, we prepare a clear and personalized care report. This report outlines the recommended support plan, nursing and caregiver interventions, daily assistance requirements, safety measures, and long-term care goals. Once approved, we deploy trained caregivers or licensed nurses experienced in physical disability support.
Medical and Support Services Provided
Our nurses and caregivers provide structured medical care and daily living assistance, including:
Mobility and Transfer Assistance
Support with walking, wheelchair use, and repositioning
Safe transfers between bed, chair, toilet, and bathing areas
