Palliative care is a specialized branch of medicine focused on symptom control, quality-of-life optimization, and supportive interventions for patients with life-limiting or chronic progressive illnesses. Palliative Care Process
The process begins with a referral or direct intake from a patient, family, or physician. During intake, our team collects detailed clinical history, including primary diagnosis, comorbidities, stage of illness, current pharmacotherapy, laboratory results, imaging reports, and prior interventions. Functional status, cognitive capacity, and psychosocial context are also assessed to develop a holistic care plan.
A comprehensive palliative assessment is conducted by licensed nurses and clinical coordinators to evaluate:
Pain severity and neuropathic/visceral pain characteristics
Dyspnea, oxygenation, and respiratory function
Gastrointestinal symptoms (nausea, vomiting, constipation, anorexia)
Fatigue, cachexia, and nutritional status
Cognitive and neurological function, including delirium or anxiety
Skin integrity and risk for pressure ulcers
Home environment and caregiver capacity
Following the assessment, a structured palliative care plan is formulated, outlining pharmacologic and non-pharmacologic interventions, nursing protocols, monitoring frequency, escalation pathways, and patient/family education. Care is then delivered by registered palliative nurses, nursing assistants, and trained carers with expertise in complex symptom management.

Pain and Symptom Control
Administration and titration of analgesics, including opioids, NSAIDs, adjuvant neuropathic agents, and muscle relaxants
Management of dyspnea with oxygen therapy, nebulized bronchodilators, or opioid titration
Control of gastrointestinal disturbances using antiemetics, laxatives, and prokinetic agents
Management of terminal secretions and oral care using suctioning or glycopyrrolate as indicated
Vital Signs and Clinical Monitoring
Regular monitoring of blood pressure, heart rate, respiratory rate, SpO₂, and temperature
Detection of hemodynamic instability, infection, or decompensation
Early reporting of clinical deterioration or red-flag symptoms to supervising physicians
Medication Administration and Complex Regimens
Adherence to prescription orders, including syringe driver or infusion pump therapies
Monitoring for adverse drug reactions, polypharmacy complications, and therapeutic efficacy
Documentation of medication response and titration outcomes
Mobility and Pressure Ulcer Prevention
Repositioning using pressure-relieving devices and appropriate body mechanics
Skin assessment using Braden or Norton scale for risk stratification
Application of barrier creams and specialized dressings
Nutrition, Hydration, and Metabolic Support
Monitoring oral intake, hydration, and electrolyte balance
Support with enteral nutrition or feeding assistance when indicated
Observation for cachexia, sarcopenia, or dehydration
Psychosocial and Cognitive Interventions
Support for anxiety, depression, or delirium, using both pharmacologic and non-pharmacologic strategies
Facilitation of advance care planning, goals-of-care discussions, and family counseling
Assistance with spiritual care and end-of-life decision-making
Documentation and Multidisciplinary Coordination
Detailed recording of symptoms, interventions, response to therapy, and patient/family feedback
Communication with primary physicians, specialists, and hospice teams
Regular review and modification of care plan based on disease progression and patient status
