Nursing Assistants
Nursing assistants are essential members of the healthcare delivery team, providing direct patient care and supporting clinical outcomes under the supervision of licensed nurses. At Andella Home Care Services, our Nursing Assistant Services deliver structured, medically informed care to clients in home-based and long-term care settings.
Our Clinical Care Process
Care begins when a client, family member, or healthcare professional initiates a referral. During the intake process, we collect relevant clinical information including medical diagnoses, comorbidities, functional limitations, medication regimens, and physician orders.
A comprehensive nursing assessment is then conducted to evaluate:
Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
Mobility status and fall risk
Vital signs baseline
Skin integrity and risk of pressure injuries
Continence status
Nutritional and hydration needs
Based on these findings, a patient-specific care plan is developed, detailing the scope of nursing assistant interventions, frequency of care, documentation requirements, and clinical supervision structure. A qualified nursing assistant is then assigned accordingly.
Clinical Duties and Medical Responsibilities
Our nursing assistants perform evidence-based, hands-on patient care, including:
Vital Signs Measurement and Monitoring
Measurement and documentation of blood pressure, pulse, respiratory rate, body temperature, and peripheral oxygen saturation (SpO₂)
Recognition and reporting of abnormal physiological parameters
Activities of Daily Living (ADL) Assistance
Support with bathing, dressing, grooming, oral hygiene, and toileting
Maintenance of patient dignity and infection prevention standards
Mobility, Transfer, and Positioning Care
Safe patient handling techniques for bed mobility, transfers, and ambulation
Use of gait belts, walkers, wheelchairs, and other assistive devices
Repositioning schedules to prevent pressure ulcers
Skin Integrity and Wound Observation
Routine skin assessments to identify erythema, edema, or breakdown
Monitoring of pressure points and early-stage pressure injuries
Assisting with non-complex wound care as directed by a nurse
Elimination and Continence Management
Assistance with urinary and bowel elimination
Monitoring intake and output (I&O)
Perineal care and catheter site observation (where applicable)
Nutrition and Hydration Support
Assisting with oral feeding and dysphagia precautions
Monitoring nutritional intake and hydration status
Supporting prescribed therapeutic diets
Medication Support (Non-Administration)
Assisting with medication reminders and observation of self-administration
Monitoring for adverse drug reactions and reporting promptly
Infection Prevention and Control
Adherence to standard precautions and transmission-based precautions
Hand hygiene compliance and use of personal protective equipment (PPE)
Environmental sanitation of patient care areas
