Carers & Personal Aides
Carers and personal aides provide essential non-invasive, supportive care that promotes functional independence, safety, and quality of life for individuals requiring ongoing assistance at home. At Andella Home Care Services, our Carers & Personal Aides Services are delivered using structured care plans, health monitoring protocols, and person-centered support approaches under appropriate clinical oversight.
Care Intake and Assessment Process
Service delivery begins with a client or family-initiated referral. During intake, we obtain relevant health and functional information including primary medical diagnoses, physical or cognitive limitations, mobility status, continence needs, nutritional requirements, and psychosocial factors.
A functional care assessment is then completed to evaluate:
Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
Mobility, balance, and fall risk
Cognitive status and behavioral considerations
Skin integrity and pressure injury risk
Home safety and environmental accessibility
Based on the assessment findings, a personalized care plan is developed outlining daily care tasks, level of assistance required, risk management strategies, and documentation protocols. Trained carers or personal aides are then assigned accordingly.
